Patient Code of Conduct
We kindly ask all patients and clients to treat our staff and each other with courtesy and respect. Harassment, raised voices, aggressive tones, or inappropriate comments will not be tolerated.
Credit Card
To confirm your appointment, we require a credit card on file. Your information is securely encrypted and stored in our system.
Cell Phones
Please silence your cell phone while visiting the clinic and take any calls outside.
Scent-Free Policy
For the health and comfort of others, this clinic is a scent-free environment.
Inclement Weather Policy
During periods of inclement weather, we kindly ask that patients remove outdoor footwear upon arrival to help keep our clinic clean and safe. Slippers or disposable booties are available for your comfort.
During these conditions, we also ask that strollers not be brought into the main clinic area. Designated stroller parking is available in the small waiting room to the left of the entrance.
New Conditions/Accidents
Please let us know if you have a new condition or recent accident so we can schedule the appropriate appointment length for you.
Multi-Disciplinary Clinic
We offer multiple wellness services. Please complete a separate health history form for each service, as intake forms vary.
Courtesy Reminders
We offer email and/or text reminders for your convenience. Please let us know your preference and be sure to mark your calendar.

Missed/Cancellation Policy
To help keep care accessible for everyone, we kindly ask for 48 hours’ notice to cancel or reschedule appointments. This policy applies to all services except chiropractic.
- For Monday appointments, please let us know by the previous Friday if you need to cancel or rebook.
- No-show or missed appointments are charged 100% of the appointment fee, as we are unable to fill that time.
- Late Cancellations:
– First late cancellation is waived once as a courtesy for new patients
– Second late cancellation: 50% of the appointment fee
– Third and subsequent late cancellations: 100% of the appointment fee

Insurance
We are happy to email you invoice statements for your insurance or tax needs at any time. We do not offer direct billing to personal insurance companies.

Electronic File Consent
I understand and agree that my health information will be maintained by practitioners of Back in Balance in an electronic form and may be electronically transmitted to other health care providers, insurance companies, or myself as required.
